State Law Require New Forms to Be Completed

Due to recent changes per Tennessee’s Public Chapter 1061, WCS families must complete some new forms before their student can have access to certain school services. Those include basic, non-emergent medical care and school counseling.
The Consent to Treat Form gives school employees the ability to administer non-emergent medical care. If consent is not given, that student will not receive non-emergent medical care, which may include basic first aid such as cleaning and bandaging minor cuts or abrasions, providing an ice pack and managing injuries. Consent will remain in effect until withdrawn in writing by a parent or guardian. Per State law and WCS Board Policy 6.405, no medication will be given in the school setting unless medication is provided by a parent or guardian and appropriate medical authorization is on file.
The Consent for Counseling Form allows students to receive school counseling services. If consent is not given, that student will not receive any counseling services typically provided building-wide, which may include but is not limited to academic advisement; college and career counseling; safety and well-being presentations; quick check-ins for grades or missing assignments; or any support requested by the student. Selecting "yes" to the opt-in process does not give school counselors permission to provide group or individual therapy. Both of these require additional parent communication and permission.
Student surveys also require parents to opt in. Each survey permission form will have a list of the questions for preview.
Families are encouraged to take a look at their Skyward Family Access page to see if there are any outstanding forms that need to be completed. If you would like to change the consent indicated on either form, contact your student's school secretary.
If your student will not be attending WCS this school year, please contact the school directly to take the necessary steps to complete the withdrawal process.
